Connect & Learn: increasing referrals to the National Diabetes Prevention Programme

Connect & Learn is a series of online learning events designed to share and spread improvement across the North East and North Cumbria health and care system. These sessions go beyond information-sharing, creating space to connect with colleagues, generate ideas, and inspire practical improvements in your own role, team or service.

The next session, Increasing Referrals to the National Diabetes Prevention Programme (NDPP), will take place on:

Supporting prevention and population health

This interactive session will explore how primary care colleagues can maximise the impact of the National Diabetes Prevention Programme and support more people to access it.

Participants will gain a practical understanding of the referral and onboarding process, helping to build confidence in identifying and supporting eligible patients. The session will also showcase a real-world case study from a Primary Care Network (PCN), highlighting how Health and Wellbeing Coaches and social prescribing teams have been used to increase referral rates and improve retention.

Colleagues from Reed Wellbeing will also share how they are supporting practices and PCNs to strengthen collaboration and enhance patient engagement.

Connect & Learn sessions follow a simple structure to support shared learning:

  • An introduction to the topic and why it matters
  • A guest speaker presentation
  • A facilitated discussion on what this means for our system

This approach helps participants reflect on their own practice, explore new ideas, and consider how learning can be applied locally.

These sessions are open to all staff and volunteers across the North East and North Cumbria health and care system, particularly those interested in prevention, population health and improving pathways for people at risk of long-term conditions.
