Working together to transform neighbourhood health...

Health and care partners across the North East and North Cumbria are invited to take part in the first wave of the National Neighbourhood Health Implementation Programme (NNHIP) — a bold new initiative designed to reshape how care is delivered in our communities.

This is a unique opportunity to build on the fantastic work already happening across our region and to help shape a more joined-up, preventative, and person-centred approach to care at neighbourhood level.

What is the NNHIP?
The NNHIP supports local areas - known as ‘Places’ - to develop neighbourhood health systems that focus on people with multiple long-term conditions and rising health risks. The aim is to:

  • Shift care closer to home
  • Reduce pressure on hospitals
  • Improve outcomes for people and communities
Places selected for the programme will benefit from:

  • A dedicated national coach
  • Access to regional learning networks and workshops
  • Practical tools, resources, and best practice examples
  • A chance to influence future national policy
