All people with a learning disability age 14+ are entitled to a learning disability annual health check.

Introduction

Annual health checks are delivered in general practice and, potentially, the most important tool/opportunity we have to reduce premature mortality and address health inequalities. After a health check has been completed the person will be given a health action plan to support them to improve their health.

Alongside partners of the network several resources have been developed to support healthcare staff and people with a learning disability their families and carers with annual health checks.
