A personalised care definition:

Personalised care means the planning and delivery of care based on what matters to the individual and their own strengths and diverse needs. We are here to provide the tools and training which will enable staff and volunteers across the North East and North Cumbria Integrated Care System (ICS) to develop and deliver their Personalised Care skills whatever role they play in a service user’s journey.

What is personalised care?

Giving service users choice and control over their own care and health depending on what their strengths, needs and goals are.

The whole system approach integrates services from health, social care, voluntary sector and the wider community to enable the best outcomes for our population.

Making Every Contact Count (MECC)

MECC is an approach to behaviour change that utilises the millions of day-to-day interactions that organisations and people have with other people to encourage changes in behaviour that have a positive effect on the health and wellbeing of individuals, communities and populations.

It is simple signposting to better health and wellbeing. Helping you to have brief conversations in order to raise awareness, motivate and signpost people to better health and wellbeing using three steps:

Ask – take notice and engage
Assist – provide motivational health information
Act – signpost to a local service

MECC Training

Personalised Care - Conversations and Shared Decision Making
The course will cover an introduction to what personalised care is and the skills needed to begin embedding personalised care conversations and shared decision making in health and care.

It will challenge practice with the aim to changing mind-set of workers and volunteers so that personalised care becomes part of everyday practice. This will enable staff to also share best practice and will therefore impact positively on service user and patient outcomes.

AIMS

  • Learn about how coaching skills can improve delivery of personalised care
  • Learn about support and services available to signpost to
  • Practice embedding personalised care conversations in your role
  • Look at tools to aid with personalised care
  • Utilise the 'three talk model' of shared decision making when having personalised care conversations

Communities of Practice Network and Training Events


Events throughout the year that will provide a mixture of group and workshop sessions, with some virtual training to follow. Learning goals:

  • Opportunities for networking and connection with others in similar roles
  • Opportunity to meet third sector organisations who help support service users in their communities
  • Soft skill training so staff can have more skilled and meaningful interactions with service users
  • Education and awareness, referral routes and links to support services
  • Sharing best practice across teams
