Brought to you by Boost in collaboration with North Cumbria Knowledge and Library Service, this LUNCH & LEARN session on 7 October 2025 saw Lindsay Oliver and Helen Kleiser (NHS Year of Care Partnerships) deliver a presentation on ‘Proactive care: Developing a personalised proactive care toolkit with PCNs in North Cumbria’.
In this session, we heard about the proactive care toolkit which summarises the Year of Care approach, learning and resources for teams looking to implement or enhance proactive care in their area.
The toolkit was developed to collate the learning from working with two PCNs in North Cumbria who utilised integrated neighbourhood teams to deliver a model of proactive care. The model was based on the Year of Care approach to personalised care and support planning for people with multiple long-term conditions and frailty.
