Led by Dr Su Ann Tee and Jonathan Fenwick, the service is the first in the UK to offer a fully integrated clinic supporting people recovering from heart attacks, strokes, TIAs, angina and circulation problems. Since 2023, the team has brought together specialists in cardiology, kidney care, diabetes, stroke and pharmacy to create personalised treatment plans that help patients manage cholesterol, diabetes and blood pressure more effectively.

By coordinating care in a single clinic, the service improves outcomes, supports medication adherence and provides better value for the local health system. Judges – who reviewed more than 1,200 entries – praised the team’s ambition, collaboration and clear impact.

Dr Tee said the award reflects “the power of collaboration” and acknowledged the wider multidisciplinary team.
