
A new tool called the Family Voices Diary is helping healthcare professionals better support family caregivers during the final days of a loved one’s life. Co-produced by the Specialist Palliative Care team at North Tees and Hartlepool NHS Foundation Trust, the diary was developed in collaboration with bereaved carers, healthcare staff, and members of the public.
This simple, low-cost diary provides space for relatives and friends to reflect, communicate with staff, and access helpful information about end-of-life care. A recent study, supported by Applied Research Collaborative (ARC) North East and North Cumbria, evaluated its use across hospital, hospice, and home settings in Hartlepool and Stockton.
Families reported that the diary helped them feel heard – particularly in hospital environments – and improved communication with both staff and other family members.
