Using Clinical Digital Resource Collaborative (CDRC) data searches, pharmacy teams identified patients with uncoded CKD indicators and provided targeted reviews, education, and appropriate medicines. Patient Group Consultations further supported self‑management and understanding of CKD.

Key results

• 332 new CKD diagnoses – a 19% increase in CKD register size
• Local prevalence improved from 4.04% to 4.71%
• 435 pharmacist reviews and 509 technical reviews completed
• Significant uptake of cost‑effective medicines: ACEI/ARB (24 accepted); SGLT2 inhibitors (8 accepted; 229 started in diabetes/CKD cohort)
• 33 patients attended group consultations, reporting improved knowledge and confidence
• Clearer CKD pathways and improved staff capability across practices

The project demonstrated strong cost-effectiveness, with early interventions delivering £5k–£18k per Quality Adjusted Life Years (QALY), supporting the case for proactive CKD management in primary care.

Although there are no current plans to scale the project, the model is replicable with time, training, and appropriate funding.
