
NHS Year of Care Partnerships deliver accredited training and development programmes which support organisations to embed person centred care into routine practice. They are a team of nationally recognised clinicians and experts in person-centred care, including personalised care and support planning, support for self-management and shared decision making. Their training, resources, facilitation and support are designed to engage leaders and teams in the fundamental ethos and philosophy of personalised care, alongside the communication skills and practical organisational changes to implement in practice.
Year of Care trains multi-disciplinary teams and combines this with process mapping, facilitation and an extensive range of resources to support systematic implementation of change and integration into routine care.

Join this session to hear about the proactive care toolkit which summarises the Year of Care approach, learning and resources for teams looking to implement or enhance proactive care in their area.
The toolkit was developed to collate the learning from working with two PCNs in North Cumbria who utilised integrated neighbourhood teams to deliver a model of proactive care. The model was based on the Year of Care approach to personalised care and support planning for people with multiple long-term conditions and frailty.
