Health and care partners across the North East and North Cumbria are invited to apply for the first wave of the National Neighbourhood Health Implementation Programme (NNHIP) – a new national initiative designed to transform how care is delivered in our communities.
This is a fantastic opportunity to build on the great work already happening in our region and help shape more joined-up, preventative, and person-centred care at neighbourhood level.
The NNHIP supports local areas – known as ‘Places’ – to develop neighbourhood health systems that focus on people with multiple long-term conditions and rising health risks. It’s about shifting care closer to home, reducing pressure on hospitals, and improving outcomes for people and communities.
Places that join the programme will receive:
Applications must be a joint effort across the local health and care system. This includes:
Each Place team will need a full-time Place Coach and Project Manager, supported by senior leaders from the ICB and Local Authority. Applications must be signed by all key partners.
If your Place is selected, you’ll commit to:
You’ll also be supported to develop the tools and systems needed to make neighbourhood health work – like data sharing, digital tools, and new ways of working.
